  • Policy Type: Required dropdown with values tied to the Policy Type—“I” (Individual), “G” (Group), or “P” (Partnership).
  • Tax Status & Category: These two required fields drive the logic for filtering records by Policy Category, with the combination determining the correct category code (A–F) as mapped in the Long-Term Care Rate History dataset provided by Insurer.
